Health Screening Questionnaire
As COVID-19 cases surge across the United States, G4S is launching an updated COVID-19 Health Screening Questionnaire for employees to be taken by all employees prior to arriving at work sites each day, effective immediately.
The questionnaire, which is based on guidance from the Centers for Disease Control and Prevention (CDC), evaluates if an employee is potentially at risk of exposing others to COVID-19 and is designed to help reduce the risk of exposure to staff, your families, our clients’ employees, their visitors and the general public. It takes only about 30 seconds to complete the questionnaire, and all submissions will be confidential and maintained separately from other records, with limited access to necessary personnel on a need-to-know basis only.
Your local human resources and district manager teams are fully aware of all policies and procedures related to the screening questionnaire, and plans have been made for district managers and supervisors to distribute paper versions of the questionnaire as needed for employees to complete on site if they do not wish to complete the questionnaire electronically.
Please take the following steps before reporting to work each day:
Step 1: Complete the Questionnaire
Access and complete COVID-19 Health Screening Questionnaire for Employees via one of the three methods listed below. Once completed, you will receive an email immediately, advising you of next steps:

Step 2: Get Cleared or Receive Instructions to Stay Home
Based on your answers to the questionnaire, you will either be cleared for work or asked to stay home.
a) If cleared for work, your email will contain a timestamped access pass. Please present this to the front-desk employee at your worksite with the timestamp clearly visible.
b) If based on your response to the questionnaire G4S is concerned you are at risk for COVID-19, you will receive a timestamped email asking to stay home instead, isolate yourself and to notify your supervisor that you have been directed to stay home. The human resources team will also be notified to monitor the situation and coordinate contact tracing follow-up to assess whether other G4S employees or clients may have been exposed.
If you do not have access to the questionnaire
In the event that you do not have access to this questionnaire or email outside of work or choose to not to access the questionnaire electronically, a paper version of the form will be available for your completion at the front desk of your workplace. Please ensure you maintain social distancing and follow all safety precautions to fullest extent possible if planning on completing the paper form instead of the electronic version. If you have continuing issues accessing the electronic version of the questionnaire, or if it is regularly taking you longer than 30 seconds to complete the questionnaire, please contact your supervisor immediately
